(2) Proof of Custody/Guardianship (if applicable)
(3) Address Verification (utility bill within the past 30 days of registration)
(4) Immunization Record (shot record)
(5) Withdrawal Papers, Last Grade Card, or Transcript from prior school (if applicable)
(6) Individualized Education Plan (IEP), 504 Plan, or Evanluation Team Report (ETR) (if applicable)
